Why my 99 honda civic jolted forward instead of comming to a complete stop?

i have a 1999 honda civic lx model automatic and just the other day after applying the brake the car started to rev real high and then jolted forward hitting the car ahead of me it didnt come to a complete stop...can anyone tell me the problem? thanx
